KYC Policy of FORTUNE 5

When a player register an account with FORTUNE 5 Platform, then it is compulsory for them to perform a full KYC process. You can verify their accounts through uploading the necessary documents from the Documents tab, which is located within the “My account section”.

During this process, the user will have to input some basic details about themselves and then upload:

• Proof of ID (front and back depending on the doc)

• Selfie of themselves holding the ID document

• Bank statement/Utility Bill

Once uploaded, the user will get a “Temporarily Approved” Status and the documents will now be on our side, where the “KYC Team” will have 24hrs to go over them and email the user about the outcome:

• Approval

• Rejection

• More information needed – No change in Status. Our team will contact you and request additional information.

Guideline for the “KYC Process”

When uploading your documents, please follow the guidances below

Proof of ID

Please make sure the following are applied;

• Pictures is clearly visible

• Full Name matches client’s name

• Document does not expire in the next 3 months

• Owner is over 18 years of age

Proof of Residence

As Proof of Residence we accept Bank Statement or Utility Bill that must meet the following:

• The document is not cropped and all official stamps and logos are clearly visible.

• Full Name matches client’s name and is same as in proof of ID.

• Address matches the client's address submited when the account was created.

• Date of Issue: In the last 3 months

Selfie with ID

Real-time photo ("selfie") while holding the front and back sides of the document you chose, under your chin must be taken in high resolution, good lighting, no flash used, no makeup, sunglasses, hats or any other disguise and the chosen document under your chin. Please note that the Information on your document must be clearly visible.

• Holder is the same as in the ID document above

• ID document is the same as the one uploaded by the client.

Payment Methods

The verification of a customer’s proof of payment will depend on the deposit method they choose to deposit with. The requirements will differ for each method and may need to be updated periodically due to changes the payment methods may make to their user interfaces and/or the Casinos accepted payment methods. The below is a list of current proof of payment requirements at the Casino based on the currently accepted payment methods:

Card payments

Copies of all debit/credit cards registered and used on the player’s account. These copies must display the player’s signature together with the first six and last four digits of the card itself. For the player’s own security and safety, players should black out the security number at the back and the middle digits in the front of their card.

eWallets

A screenshot of your ewallet account showing the player’s email address/account number and personal account information.

Bank Statement

A bank statement issued in the last 3 months for the player’s bank account. This should include the player’s bank name, bank account number, IBAN number and BIC code. All four corners of the statements are to be visible. The statement may be a hardcopy or a PDF. We also request a PDF or screenshot showing the player’s deposit to the Casino.

Crypto Currencies

A screenshot of transaction and a screenshot of customer’s crypto wallet profile shows e-mail, user ID and personal account information.
